  In Balsam Lake, the office is located on the ground floor of the Polk County Government Center within the Human

     Services Department.

  In Siren, the office is located on the main floor of the Burnett County Government Center.

  In both locations, signs are posted as you enter the buildings from the parking lot to direct you to our offices.
